Sometimes: You can do a crown lengthening or gingivectomy procedure (depending on how deep the cavity goes down) to allow the dentist to remove and fill the cavity.

Not necessarily: Depending upon the size of the defect you might need a root canal, but only if there is enough tooth structure left to rebuild the tooth up afterwards.
