Depends: Depends what part of body is affected: heart= chest pain, heart attack; leg=painful cold leg; brain=stroke; gut=abdominal pain, bloody diarrhea; kidney=high blood pressure.

Pain: Ischemia refers to a lack of blood flow and circulation. This causes a significant decrease in oxygen delivery to the tissues that causes the tissues to undergo anaerobic metabolism. This then secretes Lactic Acid which causes pain. So pain is the first sign.
