sometimes i start feeling weak, run down with cold hands and feet....especially if i don't rest in the afternoon...what could it be?....

Muscle Weakness: There could be so many reasons, from heart to lung conditions to peripheral raynauds phenomena. Needing rest in the afternoon indicates some sleep connection. If you had been much younger it would be interesting to rule out narcolepsy but never too late especially if you feel sudden onset sleepiness and weakness.
