Nail bed is damaged: Unless you are able to "smooth" out the nail bed, the nail plate will grow "lifted" and a fungal infection will most likely come back. Have your toenail examined by a podiatrist. After examining the nail bed, if there is a way to allow it to heal flat, the nail plate has a chance to adhere. This can be done sometimes with topical medication, surgical intervention, and in some cases laser.

Nail Fungus: This lump on the toe under the nail can be skin that has changed shape due to the fungus or possibly some of the fungus itself that is attached to the nail bed. I would recommend you see a foot and ankle specialist to begin treatment so that it does not spead to other toes or infect the new nail as it grows out.
