If its an MI: Not by itself. In the right setting, such as heart attack, elevated troponoin is a retty good indication there is coronary artery blockage. Coronary angiography(cardiac cath) is the gold standard to determine this & few other good tests are also available. Note that "heart blockage" could refer to blockage of coronary (heart) arteries or blockage in the heart's electrical activity (heart block).

Troponin: It's a very accurate test and is available across all hospitals, specially er's. It takes just few minutes to get test results. If it is strongly positive, it means blockage in heart vessels. But a weakly positive results could be misleading and therefore requires an expertise, like a cardiologist to get further evaluation to get the correct diagnosis.
