GI issue: 11 year had anal fissure, was he seen by a gastro-entorologist? Anal fissure, abdominal pain, loose stools are signs of inflammatory bowel disease (ibd). My suspicion is, he possibly has crohn's disease. Talk to his pediatrician for a referral to gi. Your son needs evaluation soon.

GI : Possible causes include cronhs disease causing his symptoms especially if he has multiple bm. Other condition that needs to be rule out is celiac disease. I advise he is referred to a pediatric gastroenterologist.
