Ultrasound dating: First trimester ultrasound is accurate within 5-7 days. Lmp dating also assumes a 28 day cycle. There is no test that can date conception more accurately than a 1 week window. Due date is based on LMP unless us off by more than 1 week 1st trimester or 2 weeks in 2nd. And it's only an estimate; the baby will decide when to come regardless of what you or your doctor think :).

Maybe: Before 5 or 6 weeks, the gestational sac that will contain the baby may be seen, but usually the fetal pole (actual baby) is seen after 6 weeks and there should be a heartbeat seen by 6-7 weeks as well. The period date can be off by as much as 2 weeks, so the sonogram is helpful and very accurate for giving you the correct gestational age.
