Not every case: Not everyone with bipolar disorder has 'psychotic features'. It is more commonly seen in bipolar type i. I frequently see the symptoms in patients who are manic but it can occur in the depressed phase as well.

It is a symptom: Grandiosity is a symptom of bipolar disorder and it can become delusional (belief that one is a god or jesus) that would indicate a split with reality. Most grandiosity is inflated self-importance and may have elements of truth tying it to reality ie, i'm a high powered executive (translation i work at a prominent company, but don't hold a title.).
