You've started: Recognizing that you want to change is a big first step. Next, stop beating up on yourself for where you are and focus on taking a step--however small--in the direction you want to move. And take credit for starting! also consider getting screened for depression--that may be undermining your motivation and treatment can really help. Small increases in activity can make a big difference.
