Depends: Most women have an anteverted uterus (tilted forward) and usually complain of cramping in the lower abdomen above the pubic area. Women with a retroverted uterus (tilted toward the spine), may have cramping more in their back. Some have both. And women with an enlarged uterus (like fibroid uterus) may have the pain and cramps even higher. The uterus sits behind the bladder.
