Yes, very rare: It is not unusual to have lyme for years before proper diagnosis. While it is possible to get malaria in the us it is extremely rare, a few cases at most each year, but would be hard to diagnose since most docs would not consider it. Babesia, a tick-transmitted parasite similar to malaria is much more common in the us. Longstanding lyme is difficult to treat - see comment for more:.
