Leave it alone: Most patients with a hemi-diaphragm paralysis do not have symptoms and therefore don't need treatment. If you start to develop pain with breathing (usually with physical exertion), the next option is called surgical plication. The affected portion of the diaphragm is sewn together front-to-back to restrict upward motion of the diaphragm. Again without symptoms this is usually not necessary.
