Possibly: Most bleeding after a medical miscarriage should be finishing up at the two week mark --at the longest usually. If the bleeding is still significant after 2 weeks then you should seek medical attention because your miscarriage could be incomplete and / or there may be an infection complicating things as well.

Yes : After a miscarriage bleeding will continue until everything that needs to come out is out. The duration will vary from person to person. Since surgery was not done in your case your doctor should be following your bhcg hormones to know when the miscarriage has completely resolved.

Yes: Whether it ended in miscarriage or went to term, you were still pregnant. After the pregnancy ends, there will be a period of from 2-4 weeks (usually) during which you will continue to bleed, although it will gradually diminish. Don't assume you can't get pregnant again until bleeding stops, however. If pregnancy is not something you want, protect yourself now.
