Sometimes: Some types of strokes can cause headaches but most headaches are not strokes and most strokes do not cause headaches.

Generic answer: In the elderly, headaches may be associated with cerebrovascular insufficiency issues, but usually, these are symptoms and have not marker of future events. Acute headaches can be associated with blood vessel obstruction, and especially bleeding intracranially of any variation. However, in a younger age group, frequent headaches more often, migraine or tension origins.
