Some alternatives: I don't understand the meaning of your last sentence; does it mean you have tried insomnia-directed cbt (as well as attention to sleep hygiene) but it hasn't worked? Sleep aids you didn't mentioned include rozerem, (ramelteon) doxepin, and various otc herbal mixtures. Also, withdrawal from some of the meds you mentioned could interfere with sleep. And, i'm assuming you aren't using alcohol in the evenings.

First please have a complete physical examination to know if you have an illness. If not, your medication should be managed by a Psychiatrist. If you are not confident in your Doctor please see another for a second opinion. Please see a Clinical Psychologist with expertise in multiple types of psychotherapy to help you and recommend supportive services and a sleep study.
