Are you sure?: External hemorrhoids usually come to your attention when you get a blood clot (thrombosis) that develops suddenly. It is typically a very painful lump right at the anus that will slowly get better over 2-4 days. Warm baths and avoiding constipation will help reduce the pain and speed recovery. If it is still quite painful then a surgeon can remove the clot in the office. If pain is better wait.

Not necessarily: You should only go to your doctor if they are bothering you. If you know you have external hemorrhoids then you should expect to have some bright red blood with bowel movements from time to time. Prevention entails making sure your bowel movements come out without a lot of pressure (high fiber diet, lots of water, avoid opiate pain medicines).
