Surgery, maybe: Small bowel obstruction caused by adhesions or scar tissue often resolves with supportive care, like draining gastric juices and air out of the stomach and upper bowel, bowel rest, and giving IV fluids. If it doesn't, surgery may be needed. Small bowel obstruction from other causes, such as hernia or tumor, requires surgery as primary treatment far more often.
