Example: It's wonderful that you have each other & you want to help. You already know that the man has to think the change is his own idea. Don't tell him what to do. Take up fitness and healthy eating yourself. Don't point it out. Be sure your own information about healthy lifestyle is accurate. If you both smoke / drink, quit together. Tell him you love the way he smells when he's just back from the gym.
