Ischemic cardiomyopa: When abnormal cardiac systolic function is present due to ischemia or ischemic infarction, we call that ischemic cardiomyopathy.

Decreased flow: Ischemia means decreased blood or nutrient flow to tissue. Cardio means heart. Pathy means disease or weakness. Therefore somone with decreased blood flow can end up with weakening of the heart muscle.
