ER is correct : With the limited information provided, i would opine that a visit to the er is considered judicious use of emergency medical services. Back pain and blood in dry heaves (hematemesis) could be benign but could also be indicative of a serious underlying internal condition (pancreatitis, duodenal perforations, etc.).
