Could be a virus: The most important element of diagnosis in cases such as yours is the physical examination with visual inspection of the genital area. If there are any lesions, sores, blisters, etc. Additional tests may be necessary, to test for conditions such as genital herpes. There are other possibilities, but the best advice is to see your gynecologist, if possible when the "painful lumps" are present.
