Have anal tear, what does occasional itchiness at the butthole mean?

Inflammation: Itchiness is a normal part of the healing process and is in part use to inflammation and histamine release. While some otc preparations with steroids such as anucort may help to ease the itching, it may also slow healing. There are many causes for a fissure. Have you discussed with your primary care physician?

Anal Itch: This can be a number of things, but often is yeast infection, healing sensations from the teat (known in medicine as a fissure); it can be nerve sensations when the raw skin is in contact with the paper you use to wipe or the stool itself.
