Patient preference: Some do well in the MBT style shoe, others do well with a wedge, when the heel has some height then you preload the tendon so it doesn't have to work as hard.

Resolve the injury: Achilles tendinitis can weaken the achilles tendon. The inflammation needs to be treated. When chronic, this can weaken the tendon and make it more susceptible to rupture. Treatment can include the use of an ambulatory boot/crutches and compression wraps. When the condition is nearly resolved, custom orthoses for your shoes should be considered.
