Skin Tags or damage: A chronic or large hemorrhoid can stretch the skin permanently. If there is no pain or bleeding and he has had this for decades then there is nothing more to worry about.

Anal skin tag: Most likely anal skin tag. Often develops from trauma during defacation. When the tears heal, a skin tag can form. Painless doesn't rule out something more serious and you should consider having it looked at just to be certain.
