See details: This is radicular pain caused by nerve compression in the lower back (sciatica). There are several approaches available including physical therapy, antiinflammatory meds and targeted injections. See an orthopedist or rheumatologist.

DDD and sciatica: Looks like you have already tried the first step: pt. Other things include pain management injections to the spine (epidural injections, nerve blocks etc.). And the last thing is time, most patients get better over several months. If these don't work and in the right patient surgery may be an option.
