Can an emg ncs be wrong or not pick up nerve damage on small branches in the cheek that are causing slightly lopsided smile, and slight cheek droop?

Yes: All tests have the possibility of false positive or false negative. Emg's are usually done to test peripheral nerves not cranial nerves as would be the case for sensation/motor function in your face.
