Heart attack pain: It can be. Most people who have had a heart attack describe an overwhelming sense of pressure in the chest that makes it difficult to breath and don't speak specifically of pain in the sense of a sharp sensation. That being said, pain is a very subjective response to an illness and depends on the person.

Often yes: It can be painless in about 25% of cases (50% in people with diabetes) but the more typical presentation is with discomfort which can become quite painful for some.
