Usually not: At 8-12 weeks it is very accurate for dating plus minus a few days. It cannot be off by a month.

Possibly: This is very early to detect lack of fetal growth. Check with your obstetrician and have another ultrasound to check for growth in 2 weeks. This really needs to be checked and other tests including an amnioncentesis may be recommended to check for aneuploidy.

No, if properly done: Properly performed and interpreted ultrasound examinations have a variability of 8% plus or minus the measured dating. Ultrasound dating at 12 weeks 3 days should have a 95% confidence range of 11 weeks 3 day to 13 weeks 3 days which is +/- 8%.
