Not too vigorously: If you have a hot, red, tender, swollen vein that would be suggestive that you have a superficial venous thrombosis, massaging it could be dangerous. Gentle upward pressure (not too vigorous) going from your foot to your hips, while you are lying down or have your feet slightly elevated, is likely to be most helpful. If your varicose veins hurt that much, it is time to see a vein specialist.

Not effective: Massage is not an effective way to treat varicose veins. It will not have any affect on the veins themselves & they tend to worsen the longer that you have them. I would recommend that you seek expert opinion on treatment options by a board-certified vascular surgeon who is an expert in varicose veins, as at your age you should consider having them treated with an intervention.
