Depends: Where are you using the heat pad? If the back, then 20-30 minutes but not at scolding temperatures. If it's the jaw, probably not a good idea if you just had surgery. Generally, it's recommended to use a cooling pad to decrease swelling (a heating pad will make swelling worse). Hope this helps.
