Both!: There is a theoretical concern that hemorrhage could happen and a natural tendency for physicians to be very conservative. That is explanation for answer #1. On the other hand this does not occur frequently. Answer #2. You are asking for more precision than can be had. Are you at risk- yes. But risk is low. Best bet: avoid raising intra-abdominal pressure too much. Use best common sense.
