Depends: If the mortise/ankle joint is well reduced, than this can be treated nonoperatively with a cast and nonweightbearing and close followup. If the ankle joint is not reduced or the mortise is widened, then surgery is recommended.

Displacement : If the displacement is greater than 2mm, it has been recommended to be operative treatment, but if this is the same fracture with a nonunion as you have mentioned previously, then with a displaced nonunion, you should seek operative treatment.
