Depend: During a preoperative evaluation the severity of the disease is match with the invasiveness/intensity of the surgery. Low, intermediate, high risk surgeries with low, inter, high risk patients. Then, the anesthesiologist will discuss risk and benefits of any plan anesthetic and monitoring. Example: patient with heart attack 2 weeks before with an emergency acute abdominal process is a high risk.

It depends: The short answer is it depends on what type of surgery and what are your medical problems. Some surgery is relatively minor (cataract surgery), some are moderate risk (abdominal surgery, orthopedics), while others are considered higher risk (heart, vascular).
