Hard to say : Most kids in this age group have rather plastic cartilage & seldom have a true break in the outer nasal structure, but they can occur.Eval by your doc could see all of bony structures are intact.

Has it moved?: Fracturing a four year old's nose is exceedingly difficult. Most of the exterior nasal bones are still very soft cartilage. If you have him lay down flat, then from the end of his bed where his feet are, check to see if the middle of his chin, lines up with the middle of his upper lip, then the tip of his nose, and finally the bridge of his nose. If all line up-its probably not fx'd.
