Many causes: There are many different causes for ankle swelling including poor circulation, congestive heart failure, kidney failure, high blood pressure etc. There are also other non-serious causes such as inactivity, sitting in a position with your feet hanging down.
The bottom line is to have him/her seek medical attention and have a complete physical to determine what is causing the swelling.
