Kidney function is estimated using various formulas, common one being mdrd formula.When the estimated kidney function is about 20%, discussion about dialysis and transplant begins. Many patients start dialysis, when kidney function is less than 10% or get preemptive transplant (avoid interim dialysis). It is best to discuss with a nephrologist.
