Not insurance based!: The operative surgeon decides whether the patient can tolerate an outpatient procedure or not. Staying inpatient or overnight after procedures is not decided directly because of medicaid or other insurances. However, some insurances don't cover the procedure itself and its up to the patient to pay for the procedure out of pocket. It's surgeon, patient and procedure dependent.

No: The payor does not dictate the length of stay, the surgeon does. A mastectomy without reconstruction is not a painful operation; most people are ready to go home the day after surgery. If you are in the hospital less than 24 hrs, it is considered an out-patient procedure. With a reconstruction, the length of stay is typically 2-4 days.
