Unless he has a : Neurodevelopmental disorder associated with cognitive delays or autistic spectrum disorder, he will not hurt himself. Give him safe toys to play with during diaper changes, not both masturbation & resisting diaper changes are normal behaviors for his age. If you're worried about his having penile pain or discomfort. , take him to his pediatrician to be examined.
