Good question: 1. Depends on the extent of depression and compression on brain 2. Also depends on location: cosmesis, in front of hair line etc. 3. Also if there is associated soft tissue injury that is concerning, like a open laceration above the fracture.

Yes: If it is a compound fracture or if the bone is depressed more than the full thickness of the skull, surgery is usually performed.
