What would cause pain on forehead scar from old injury ?

It depends: It is very unusual for a scar to be painful. If the area of pain is in the forehead, it is possible that you fractured the bone over the frontal sinus and you are having some frontal sinusitis. If you indeed did have a fracture, you should be seen by an otolaryngologist or ENT specialist to make sure your frontal sinus is ok and is not forming a mucocele.
