Causes of enlarged: Lymph nodes are infection, inflammation, malignancy. But this might not be cause of your back pain. This sounds like a ct scan finding. Ask the doctor who ordered the scan what he thinks you should do. You will need to get further tests and maybe even a biopsy, .
