Depends: Watch for fevers, vomiting, increasing abdominal pains, inability to eat. Minor muscle cramping can be normal this early after surgery. Try warm heating pad or showers if ok with your surgeon.

Maybe muscle trauma: One of the ports during appendectomy is usually placed in the left lower quadrant. During urination the abdominal muscles are tightening and the injured muscle may cause painful sensation. The other 2 ports are placed in the midline where there is no muscle.
