How do university surgeons get paid? is it just salary or salary + procedures? is private vs public school any different? ex usc vs ucla surgeons.

Salary alone.: University-employed professors are salaried and thus have no incentive to perform additional procedures unless they are clearly indicated. My understanding is that some private universities may give year-end bonuses on productivity, but most public ones do not.
