Well, there is the: Kidey back there, gotta consider a stone but doesn't sound like it. Does it increase with breathing? If so, likely musculoskeletal. Try ibuprofin but if sick, fever, getting worse consider er, if fever and uriary probles, could be kidney infection, my vote is muscle strain though, good luck.
