No real classic Gold Standard. With middle ear infection, the tympanic membrane is often cloudy with impaired mobility. Additionally, an air-fluid level or bubble(s) may be visible in the middle ear. The use of pneumatic otoscopy to demonstrate decreased mobility of the tympanic membrane is considered an important primary diagnostic method. In other words, good visual examination.
