The stimulation orally was more effective than vaginally. He must have been aroused enough to have an ejaculation. See a sex therapist for exam and treatment. He may be under a lot of anxiety and or stress.

This is difficult to understand- that he had orgasm without erection. He should be checked by a Urologist. Good luck
