Bloody discharge comes from some injured tissue. It is not normal. Your post doesn't provide enough information to speculate on a cause. It is worth having him examined.

Agree with Dr Ferguson. Possible causes are irritation from clothing, masturbation, a fall onto the area, a urinary tract infection, or more. Please see his doctor as recommended.
