Chronic smelly non-itchy pink/red rash, bilateral groin, doesn't respond to otc anti-fungals (or prescription econazole). antibacterial dial soap gets rid of smell temporarily but not rash. dermatologist says it's intertrigo. suggestions?

Yup, sounds like intertrigo. As she undoubtedly advised, ideally lose weight (assuming you are seriously overweight, which is likely) to eliminate folds of skin that prevent air circulation. In the meantime, or if weight loss isn't successful, wash with soap and water once or twice a day, and between washes keep the area dry with cornstarch or talcum powder. Good luck.
