Nortriptyline does not usually cause insomnia. Actually, it more often would be likely to cause some mild sleepiness. There is no direct contraindication to using the medication if you have a RBBB, but you should talk with your physician about the risks and potential benefits of any medication that you are prescribed. I would urge caution in taking nortriptyline in the presence of a conduction disturbance such as RBBB. The danger would be possibly developing complete AV block. The tricyclic antidepressant class of meds have been shown to cause conduction disturbances even in the absence of underlying EKG conduction abnormalities. I suggest looking into the use of safer sleep aids in your case

As with other antidepressants, nortriptyline may lead to drowsiness in some and insomnia in others (or no change in sleep patterns) at all. One cannot predict who will experience which until the patient starts taking it. When a person has bundle branch block, a doctor may monitor more closely a patient who is taking nortriptyline or similar drug, and that may involve getting more EKGs.
