In puberty as many as 1/3 of males will have nipple or breast enlargement on one or both sides. It can persist for up to 3 years before fading. For many it fades in 6 months or so. It reflects the small amount of estrogen males make and its effect on the tissues. You leave this alone. Rarely a few persist well beyond 3 years and if so, there are various treatments available.

In general, a hard pea size lump in a teenage male under the nipple can be caused by a cyst or infection or blocked gland. Rarely it would be something serious. In person exam by primary care physician is the general recommendation for diagnosis and treatment.
